- Patient had consumed a 250mL bottle of the mixture, ie approximately 235g 2-(2-ethoxyethoxy)ethanol
- Examinations:
- Serum 2-(2-ethoxyethoxy)ethanol, lactate, anion gap, pH plus normal vital signs.
- Medical treatment:
- Intubation and ventilation, 100mmol of 8.4% bicarbonate to treat metabolic acidosis followed by sustained low-efficiency dialysis at 10h post ingestion for 10h. Recovered and discharged after 33 days.
Results and discussion
- Clinical signs:
- Glasgow coma scale on admission 11 (motor [M]6). Admission complicated by delerium and aspiration pneumonia. Central nervous system depression, a raised osmolar gap and a high anion gap metabolic acidosis that was cleared with dialysis. Type two respiratory failure compounded the acidaemia.
- Results of examinations:
- Serum pH rose to ~7.4 20 hours post presentation and anion gap fell to ~8mmol/L. 2 hours post presentation 2-(2-ethoxyethoxy)ethanol serum level was ~2200mg/L falling to zero within 15 hours
- Outcome of incidence:
- Intensive care resulted in survival of patient.
Applicant's summary and conclusion
- Executive summary:
In a case of deliberate ingestion by a 79 year old male of a large quantity of 2 -(2 -ethoxyethoxy)ethanol resulting in an estimated dose in excess of 3g/kg, following intensive hospital treatment, the patient survived. The main cause of toxicity of 2 -(2 -ethoxyethoxy)ethanol is acidosis caused by rapid metabolism to 2 -(2 -ethoxyethoxy)acetic acid.
